Transition: Cultural and Linguistic Competence Checklist for Medical Home Teams

This resource was developed by Georgetown University’s National Center for Cultural Competence (NCCC) to help medical practices provide culturally responsive healthcare transition services for youth moving from pediatric to adult care.
