Healthy Transition Initiative: Issue Briefs 1-3

Youth with serious mental health conditions face significant challenges as they transition to adulthood, often experiencing poor outcomes due to services designed for children or older adults rather than their specific developmental needs. To address this gap, SAMHSA’s 2009 Healthy Transition Initiative (HTI) funded seven states to implement evidence-based service delivery models in local communities, while also engaging stakeholders, addressing systemic policy issues, and involving young adults and families in the process. This initiative aimed to improve outcomes for this vulnerable population by creating age-appropriate, community-based interventions that better align with young people’s unique needs during this critical developmental period.
This series of briefs focuses on three topics relevant to the HTI goals: 1) impact of the HTI grant funds at the local community level; 2) description of state-level policy and system changes in HTI jurisdictions; and 3) description of the the client-level outcomes produced by HTI grantees.
